For your safety and the safety of our staffs and providers, please comply with following protocol:
- If you have any chills, malaise, body ache, fever at or above 99 degree F or suffering from any upper respiratory symptoms such as runny nose, sore throat, ear ache, cough, Difficulty in Breathing or had similar symptoms over the previous 7 days, stay at home and contact office to be attend to via Tele-health
- Before your visit sign in to your patient portal and check your health record summary. Review them and update them as deemed necessary. Also complete the Provided Health Assessments. Forward the updated records and completed Assessments back to us via email (bufordcare@gmail.com) or faxed them to us at (770)271-8428. Upon arrival of the above mentioned information, you will receive a confirmation call from our staff regarding your visit.
- On the day of your visit arrive at least 15 minutes earlier than your appointment. Stay in your car and call us that you arrived. Our staffs will assist you remotely to sign in and handling your dues and balances. Be prepared to provide payments by Credit Cards only as no cash or checks will be accepted for safety measures.
- You will be asked to come to the front door for checking your body temperature. Please ware a surgical or cloth mask covering your nostril and mouth (No admission without mask). If you have a body temperature of 99 degree F or above, you will be asked to return to your call and our Provider will assess you via Tele-health.
- If you have no fever you will be asked to come in and you will be guided to the treatment room directly after your weight has been checked. Please turn off all your electronics including your cell phones to prevent interference with our electronic system.
