Insurance

Claims are submitted to all major insurance groups. When visiting our clinics, please bring your insurance card and picture ID to help us verify your eligibility and benefits. 

Patients are responsible for any outstanding balances pertaining to patient responsibility or yearly deductible. All outstanding balances must be met before patients can be seen by a provider. 

The clinics of Buford Care Inc. are ONLY responsible for checking a patient's eligibility. All other insurance plan details are solely the responsibility of the patient. These include but are not limited to deductibles, co-pays, co-insurances, and coverage for services and procedures (injections, ear irrigation, minor surgeries, bloodwork, and x-rays, etc...).

All patients are also required to update any changes in name, phone number, email address, insurance, and address as soon as the change has occurred. 

Please also remember that some insurances have different policies with complete physicals. For example, only allow physicals once every 365 days, and others allow on a calendar year basis. Please check with your insurance before scheduling an appointment
